Our office accepts the following insurances:
Blue Cross Blue Shield, United Healthcare, Aetna, Humana, Tricare Select, Viva Health, Cigna, and Multiplan.
We are not accepting new Medicare patients. We do not accept Medicaid.
It is the policy of this office to charge a fee of $30 for appointments cancelled less than 24 business hours or for any missed appointments.
It is the responsibility of the patient to inform the office staff of any cancellations or rescheduling.
Courtesy reminders will be sent to patients, but it is the patients responsibility to keep up with their appointments.
